Wondering if anyone can help me. I would like to know when will I get the work and capability form. And what does this mean.

This means you have been referred for the work capability assessment. You should receive the form by the middle of next week at the latest. There's a copy of that form here.

You can use that form and type your answers on it but you'll need a printer to print it off to send it back. Please wait until you receive the form in the post before sending it back.

There's also a guide here that may help you. Please note, the guide says ESA50 but the UC50 is the same.
