Universal credit uc50 forms

hi, I have recently compleated my universal credit forms and sent them I am expecting at some point soon to receive the uc50 forms in the post
I was wondering what evidence do I need to provide them abput my health conditions to support my forms. Thanks Sarah

Collate as much evidence that may support your inability to carry out the activities set out in the form - safely, reliably and repeatedly. This may come in the form, for example, of OT/ PT reports, Social Worker letters or even your own anecdotal evidence - diaries. Diagnostic letters would not be of interest to the health professional carrying out your future assessment as the UC50 is all about your functional limitations in a work place setting and not about your specific medical condition.

can a talking therapist give a report on mental health as part of evidence or is that not really evidence enough

Yes - a therapist report that supports your functional limitations would be useful evidence for the assessment.
