It should be the address on the top right of the first page of the decision letter.

It is possible to just have an award for one part, either Daily Living or Mobility. Each can be either Standard or Enhanced rate.
It won't generally affect ESA or UC, the DWP know PIP assessments aren't reliable.
Different addresses are used for different areas so we can't confirm which one you need without seeing it. If you can post a picture of the letter, so we can see the addresses in context, maybe we can work out which one is correct. (Make sure to cover any personal info if you do that).
