I've had to fill another form in?

I’m struggling what to do or where to get help.

I filled in the LCWRA form back in September 2023 I contacted universal credit they said just wait . Fast forward I had to fill in another one as mine must of got lost November 2024 . I still have not heard anything they say they have my form .

Is this a review or a first assessment? If it's a review there are extremely long delays and your form being lost wouldn't have helped. You can try ringing the assessment providers, if you haven't already. To find out who does them in your area please see the link.

it’s my first assessment. Thanks for the link I phoned the assessment place back in December they just said it’s been dealt with

thanks The form it self is stressful enough then to have to chase after it. Maybe this year they will sort it out
