Children’s new OT assessment

We’re awaiting a council house as we need adaptions for a disabled child. Been waiting 3+ years now and they’ve bypassed us twice now in the last 6 months blaming “computer system” error! Both times!
OT report was done over 3 years ago. And 2 houses ago. OT came out and assessed new house but hasn’t done a new written assessment. Got it today and was shocked!
Whilst end result is the same our living conditions are not at all.
Is there any regulations about this so I can challenge it?
