Uc migration

I've found found the courage to start my uc migration form online. But I'm already so confused and I've called the uc helpline and they are very uninformed. Would really appreciate some help please.
Firstly I can see a to do list which I'm trying to work my way thro. Is this the form, as I can't see a actual form? Or does the form appear after completion of the to do list?
Secondly, there's a question about council tax reduction... Please see photo I've attached
I'm confused because, I currently do have a council tax reduction. I have absolutely nothing towards council tax, I think this is because I'm either severely disabled or a care leaver, but does it mean currently? So ild tick yes? Or, am I meant to reapply for council tax after moving to uc and they want to know if I'll apply for the reduction again???
And lastly, I'm on a question which ask about stocks, share and dividends. Last year wessex water offered customers £20 off bill or some shares. Which I believe are currently worth around £2. I get about 50p backk a year. My understanding around this matter is zero and I find it so confusing. Do I put yes, or no as it's barely nothing and I don't really know the details.

What you are filling out is the UC application form.
You should answer yes to both questions about CT.
Once you've applied, let your local council know, they may ask for your first payment statement. Some local authorities treat UC income differently to legacy benefits, and some people have seen the amount they have to pay towards Council Tax increase.
As each local authority has different rules, we can't advise on individual cases from here.
As you have shares then you should answer yes, and input the value (to the best if your knowledge) if asked.
