Hi,
Not a template as such but this is along the lines of what you might want to include. Being honest is important as the medical Officer will know from the supplied medical evidence the described and accepted symptoms of your condition.
The personal statement in Part B of the NHS ill-health retirement form is a crucial section where you, the applicant, describe your condition and explain how it impacts your ability to continue working until your normal pension age. It's your opportunity to provide a detailed account of your medical situation and its consequences on your work capacity. Here's a breakdown of what to include in your personal statement:1. Describe Your Condition:
2. Explain the Impact on Your Work:
3. Demonstrate Permanence:
4. Provide a Clear and Concise Statement:
By following these guidelines, you can create a strong personal statement that clearly explains your situation and supports your application for ill-health retirement.
Good luck, hope this helps a little
