Mandatory reconsideration PIP

I want to know how to apply Mandatory Reconsideration ,do i send in a letter with all descriptors and explain as i as given all zeros after a telephone assessment for my mental health by a paramedic.Also ive been told three different addresses on the call.Can anyone help me with the correct fowarding address please

i now want to send further evidences from my Psychiatrist and Gp.
