I have a UC50 form I need to send it back but there is no envelope inside to know where to send it back? Help

Hi @Andreabryan1993, did your form come with a letter or was it on its own? Sometimes the address will be on the letter.

If not, the advice from the DWP is to look up your health assessment provider and contact them. They should be able to supply you with the address to send it in ☺️
