ESA to uc

Hi
We migrated to UC on the 27th may 25 went for id check 4th June 25. On our journal to do list hubby's side it's saying
Health : declare changes
What does this mean we filled all his health conditions on the application but I think I have pressed something I shouldn't of done.
Please help tyia
