Moving from residential care into solo accommodation: what's the process

Hi All,
We need some advice. Our daughter of 23, who is disabled (PWS) is currently in a small specialist residential care home. it has been agreed that she would benefit from being in her own environment.
Now, the process has started and had several meeting with existing support agencies and potential new vendors. The frustration we have is that there doesn't seem to be any formal process that covers each step/phase. It would be plausible that her case worker (she's on her 3rd one in 2 years) would follow some form of process that's visible to all parties. It's obvious each agency understands what is needed and what they need to do but we feel that it's not fluid enough and would love any input from anyone's experience with this so we can at least start the discussion and have it managed better and get some ownership in place.
