Help with health assessment forms

Hi to everyone in the Scope Community.

For my ESA assessment in 2019, I was receiving therapy for my mental health conditions (this was in the form of being referred by my GP to an Occupational Health Therapist), so I entered their details on the part of the form listed as "About other Healthcare Professionals, carers, friends or relatives who know the most about your disability, illness or health condition".

Things did not go too well regarding this therapy; my original therapist left, and the interactions I had with her replacement were not successful.

I was eventually referred back to my GP, who prescribes me various medications for my conditions.

Now in 2026, and having migrated over to Universal Credit, given I no longer have a therapist (I did not inform DWP that the therapy had ended as it was a referral via my GP and things haven't changed for me), when the time comes to be reassessed who could I turn to as a suitable individual who knows the most about my health conditions.

And given ESA is being phased out, is it likely there be a new form?

I hope the wise and discerning members of this forum are able to help, as I am quite anxious about the way this government is portraying those with mental health conditions, and what reforms may eventually be coming down the track.

    You could put a family member or friend down.

    Otherwise leave it blank or repeat your GP details if there isn't anyone else.

    The current form is WCA50, this was formally introduced a few weeks ago. As I understand it this is broadly identical to the previous ESA50/UC50 forms.
