Pain Killers

I have had decades of constipation/diahorrea – I have had cameras up my bum, I have had scans etc but no cause has ever been found for it. I have been on various drugs to try to cure it but each has it’s own unwanted side effect.
About 20 years ago, I was prescribed codeine – this worked.
Now, come forward a couple of decades and my diahorrea has become constipation, which is caused by codeine but the pain killer is needed because of knee, and intense shoulder pain.
There is no alternative - it's either constipation or pain - which is worse ?
