I am looking for the PIP Assessment Form on line. Also can anyone tell me if you can get cheaper rail fares when you have a Blue Badge Permit

You need to call the DWP to start your claim, please see https://www.gov.uk/pip/how-to-claim
With regards to cheaper train fares and the blue badge scheme the answer is no. Please check out http://www.disabledpersons-railcard.co.uk/are-you-eligible/ to see if you are eligible for a disabled persons rail card.

@Geoark is right in that you can't do the PIP forms online, you need to call them and then they send out a form.
