I have complex health issues I won my dla tribunal for indefinitely middle night rate care. I have  to  fill and send my  pip form  back. I was hoping for someone to check my statement  ( my additional  information )
Whether if it sounds  okay and because am dyslexic

Hi @usha, I would definitely contact @BenefitsTrainingCo or even give a phone call to scope number to arrange perhaps meeting with advice. 
Otherwise please speak to CAB adviser they can check the form with you together too.

Basically  I was born without  a bottom  hole or wee I have had multiple surgery's in my life. I had my bladder  made from  my bowels and I catheter every  4 hrs  24/7. I only have 40  percent  kidney  function.  And recently  I have jus found out I will  need to have surgery in my vagina in order if I want a active sex life.  I also suffer from chronic depression  and anxiety.

Hi @usha

Welcome to our online community/website.

I have also had bladder/bowl surgery 3ys ago now.

This enables me to catheterise through my bellybutton.

This has made things much worse as it leakes & it is now blocked meaning more surgery thais friday !!!!!!!

Please please let me know if you want someone to  help you with the pip form.

We have got a "Benefit Calculator" on our website which will enable you/us to check it beforehand ???
