    Hi trishad and welcome

Have you asked for a copy of the assessment report ?
You have one month from the date on the decision to request an MR

Was this a review or a change from DLA to PIP ?

It is always a good idea to get f2f advice from CAB or similar

You may not get the report in time to help with your MR, but ask for it before you request the MR. As long as you request the MR before the month is up you can do so by phone but follow up with a letter. If you haven't received the report in time then you can say that you will be submitting further evidence and ask that they wait for you to submit it, they may wait or may not.
You should be prepared to appeal to the tribunal where success rate is currently around 69%
