Hello @stepheng81 and welcome to the community. Good to have you with us.
I'm not a benefits advisor, but we have some excellent people on here who are and who will respond to your question as soon as they can. Bear with us, please, and they will get back to you as soon as possible.
Warmest best wishes to you,

Hi stepheng81,
If you need the whole form filling in you'll need to see what services are about in your area - contact your local CAB or Welfare Rights team for assistance.
If you are stuck on one or two questions we could possibly advise on here.

@stepheng81,
It will be a difficult transition from DLA to PIP as the accessment criteria for the two benefits are totally different from each other. Whereas DLA focused on your disability, PIP focuses on your abilities to manage in your daily living and your mobility issues. Having high rates in DLA is no guarantee that you will be awarded the same in PIP. To find out how you may fair in the PIP accessment take a self test on the site work and benefits, answers honestly as the answers you write in the PIP application form have to be justified in your face to face accessment.
