keeping a diary

i had examples of someone who kept a diary of how their conditions/disabilities affected them on a daily basis but cannot find them , I want to send a "negative diary" to the HMCS as my conditions have got worse and i would like an example of what to write/how to write a diary.can anyone tell me where i can find an example .~?

has anyone mentioned in threir appeal that they have been in receipt of DLA for 19 years prior to PIP assessment and that they were told they would not have to attend any future assessments at a previous tribunal.i mentioned this -should i say so again.0

Disability Rights UK site has a draft diary that you can adapt.
However, Tribunal can only consider how your disabilities were at the time of your PIP application, not as they are now if they have got worse since.0
