hi I have received an assessment form and cannot get the right  words in my head I need help filling it in but cab can’t help anyone?

Evidence will be needed to support your claim. No one can help you fill out the form on an internet forum unfortunately because no one knows how your conditions affect you.

If you're still not sure what to do then try and search google to see else is near you, welfare rights maybe.
