Looking for the address to send a letter from my carer?

Hi,my name is lindley and would like some advice please.Could you tell me the proper address to send a letter they wanted from my carer who is my 19yr old daughter.I live in Belfast N Ireland.I spoke to some guy on the phone and he stated a letter from my girl was what they needed as she is down on my forms as being my carer which she is.I don't trust these people after all I have read about them.
