Where to begin?

I have a 20 year old serve autism and mr.. he don’t leave house he washes his hands for hours I don’t know how to get control over that or where to begin. It seems to me his autism is more than when he was in jr high school he hasn’t given me a hug in years and he use to be so loving any advice please
