Hi, I'm brand new on here, so be gentle with me

Does anybody know where I can get a copy of the PIP Renewal form? Not the first application form but the one they send when it is time to reapply/renew?
many thanks

HI and welcome,May i ask your reason for wanting this form? Have you recently sent off the form and forgot to keep a copy for yourself or are you expecting your review soon and are waiting for the form?
