    It usually means that you have reported a change of circumstances which has prompted a review (as distinct from DWP starting a review as the existing award approaches the end date).

    Thanks for info we received the form today and basically says making sure your award is right? This is obviously because we asked for a reconsideration on original claim as the award was only given for moving around 8 points and 6 points for daily living, but evidenced medication change and hospital referral in the reconsideration letter since first applying? Should I still appeal or just fill in the review form? All help appreciated thanx Michelle

    I'm not entirely clear.
    Are you saying that you have had a decision and requested a Mandatory Reconsideration and are still waiting for a decision on theMandatory Reconsideration?
    What is the end date of your existing award? 
    Have you recently provided any additional information to PIP?
