DLA form not received

My son phoned for DLA in 2020 but the form was not sent. He just told me. Can we will still ask for the assessment ?

    Was this a DLA claim for a child? Why has your son waited this long before making enquiries?

    If he’s over 16 it will need to be PIP. 
    I’d imagine you would have to start the claim over again after this long, but ring them and ask, can’t hurt.
