PIP Ending date

Hi I'm wondering what to do when my PIP ending date is coming up and my circumstances have not changed, I done a search but couldn't find anything so any information would be really appreciated.

    You should be sent a review form by PIP about six months before the end date (or if the existing award was by tribunal a letter inviting you to reapply). If you are nearer the end than six months and haven’t heard anything ring PIP and ask them about it.
