Hello, I was going to have a face to face assessment but I’ve changed it to phone appointment due to my health conditions but I received a letter for Centre for Health and Disability Assessments and to give extra information online which I’ve done already. But I’m a carer and I have health conditions what are the chances of me getting accepted?

Being a carer has nothing to do with the work capability assessment. To receive the extra money each month you will need to be found to have LCWRA. No one can tell you whether you qualify for this because we don’t know how your conditions affect you.
You can see the descriptors here for LCWRA https://www.benefitsandwork.co.uk/universal-credit-uc/uc-faq/limited-capability-for-work-related-activity0
