How do i find the pips claim form

Hi @andrewdawson - & welcome to the community. You ring the DWP (0800 917 2222) to obtain a claim form, which you can see here: http://sthelenscab.org.uk/wp-content/uploads/2020/03/Guide-to-completing-your-PIP2-form.pdf

You can also write to request a new claims form although this is extremely unreliable and even if it gets sent quickly, can unnecessarily pro-long your claim.
