These are not new forms they have been the same for quite a few years. There's a guide here that may help you.
You should treat the form as a new claim and put as much relevant information as possible. Even if there's been no changes, you shouldn't just put no change without any extra information.
You should also include a couple of real world examples of exactly what happened the last time you attempted each descriptor that applies to you. Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.
Any additional evidence you have should be sent with the form. You can ring to request extra time to return it and they will give an extra couple of weeks usually.
Please make sure you include your name and NI number on everything you send. Finally, be prepared for a long wait, anything up to about a year, sometimes longer.

If you Google pip1043 you will see mentions of the form on here and other forums going back several years.

That's an old review form I remember having one like that back in 2016. I think that's better than the one I had last year because that one seems to confuse a lot of people and so many people assume that if there's been no changes, that's what they should put.
The same advice I gave in my first comment still applies.
I know there's quite a few members here that are currently in the process of filling out the review form. It would be interesting to know which one they received.

so... I haven't had a review yet so I can't compare it to that version. But if I remember correctly I found it on rightsetnet and someone mentioned that it's a new version from November 24 :)

Hello Everyone,
Rightsnet, which is a forum made up of welfare advisors have posted about the new AR1 form which the DWP have started sending out for PIP reviews since November 2024.
See link to their thread:
Discussion: New PIP Award Review Form - RightsnetIt seems to ask more guided questions than the previous PIP review form, which was a shorter version that simply asked for any changes.
Here is a link to the new form, version November 2024:Personal Independence Payment Review
