Is it worth Contesting the Assessors Report (pip review) prior to a decision being made? Has anyone had a positive outcome from doing this please?

You would be best talking to a benefits advisor. It's mainly to do with timing as the DWP may already have the report. You can call PIP and put in a complaint about the assessment. This is a hard one to answer.

Unfortunately yes. Most of the details were recorded correctly in the FH part of the form but then it didn't correspond with the descriptor points :(
Apparently a decision has been made before I had chance to send my letter off.
Just waiting for it to arrive and then it will be a MR :(

I literally wrote it all out ready to send special delivery the next day and then got the text 'your review is completed' the bext day :(
