Recently I contacted by post CAPITA regarding change PIP assessment location from Assessment centre to my home, also asked copy of consent form because I wish record PIP assessment.
PIP Assessment guide 1.6.57 - 1.6.60
I received letter with new assessment date and time (they agreed change location), but I not received consent form, also they not provided any information why this form not enclosed.
Please let me know how long I can read consent form during assessment, because I still not received this form?
Assessor should leave a copy of consent form to me? If no, can I take copy/photograph of consent form where I signed?

@rbz - If I remember correctly from my assessment (and I might not because my memory's not the best) the assessor has two copies, one for you and one for them...

Can I read 10 minutes or more?
I asked this form in advance, because wanted to carefully read before assessment.
But still not received.

I’m not sure whether they send then out in advance. You would need to sign it on the day.I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.If i see a question that i know the answer to i will try my best to help.
