I've been handing sick notes in for months, but still haven't received UC50 forms. What can I do?

Hello 
I have been handing in sick notes for months now back to back due to a health problem and I still haven’t received any UC50 forms. Iv been writing it in the journal, speaking to the work coaches asking what’s going on and it just doesn’t seem like they are pushing anything? The last person I spoke to last week on the phone ( who wasn’t my work coach as they was sick ) was like  ‘oh you should of got them by now let me chase this up as it looks like you haven’t even been sent forward on the system here’ I was abit taken back by this was like erm ok I’m confused here. I wrote in the journal again today asking for a written update.
Getting pretty upset with all this. Just wondering what I can do?

    The UC50 should have been issued 29 days after you notified them of your health condition (or immediately in some cases). Unfortunately this seems to be an increasingly common issue and there isn't much you can do except keep chasing as you have been.  Beyond that you can try contacting your MP for help.
    Information I post is for England unless otherwise stated. Rules may be different in other parts of UK.

    That’s what I thought! Unfortunately this hasn’t been done yet! :-( I guess I’ll keep on chasing like I did today by writing in the journal. I rang up citizens advice for some guidance and they suggested written evidence is the best.

    Hi and welcome 

    Your work coach should have referred you for wca and this will trigger the uc50 form to be sent 

    Hopefully the other coach you spoke to has done this for you 

    If not then contact your job centre and ask to speak to someone more senior

    Hey Jane thanks for your reply! That’s what I’m hoping that his done it! I wrote in the journal today asking for a written update. Now Waiting on a response. It feels like they are all really bit slow and it’s getting rather frustrating.

    I can understand that 

    Journal entries are not always best and get ignored try and flag them as a payment or service issue query as then they get seen by case managers 

    Or as advised call your job centre
