Sick not

hi there
I'm wondering if anyone can advise
I did my managed migration application, I am a carer who is also disabled.
On getting help doing the online application I filled in my health conditions. I was verified as a carer
I was then asked to provide a sick note.
I have not filled in dates of sick note because it does not have it. On fit for work
It states indefinatly
The date I was assessed is day I wet to gp.
I explained in my journal to work coach that my fit note has no dart and end date .

Can anyone advise. It only has the date of assessment on which is day I got my gp appointment.
I don't want to get into trouble putting a date . I am long term sick on pip
I would be grateful for any advice.

Hi @jen1827 it sounds like you have quite a unique circumstance. The response you got is a little confusing and doesn't really seem to clarify what you need to put there.
To be sure (because it's important you put the right information in that they won't dispute later), personally I would go back to them to reiterate you don't have an end date, because it states indefinitely, and ask what date you should put because you don't want to put an incorrect date?

Thankyou so much for your reply and this is making me more unwell.
I know you cannot get both carers and health as one will cancel the other out.
The way he answers is for us to consider the health element , where you can only get claim one or the other makes me feel it's optional and would not make a difference as they have verified me as a carer with carers allowance
I feel what's the the point
I dont feel there's anything to gain , but more stress

Hello jen, the sick note is now called a 'fit note' - a Statement of Fitness for Work.
The start date is the date of your GP's signature. Your GP has advised that "you are not fit for work".
Your GP has deleted 'will' leaving "I will not need to assess your fitness for work again at the end of this period". That's clear enough for DWP.
Report your fit note ending with a date in five years' time.

hi Holly I just thought I would share the fit note so you can see what I mean about the dates and also the Gp has not put a signature on it just his dr name and practice
Hope this help you see a clearer picture as I feel I don't always explain properly
