telephone assessment for PIP re-assessment

    I have just filled in my re-assessment pip form , it is not as long as the original form .

    It tells me not to send previous GP letters in , but new condition proof .

    On the the original form I asked for a phone call assessment as I cannot look people in the eye and I cannot mix with people ,I also wear hearing aids and have profound hearing loss in both ears , I get embarrassed if i talk to people and i have to ask them to repeat what they said and i get anxious that people will become impatient with me , i also try to lip read and i need to be within 6 feet of anyone i speak to , its difficult for me . I did get a phone assessment and what was called paper assessment (I think that is what it is called)

    So do I say again on this re-assessment form that I need a phone assessment ?

    PS the questions are the same but not as many as the original form (i think) lol .

    I have an occupational therapist assessment report and it includes all the NHS aids I have been given . I was told by MacMillan that this needs to be photo copied and sent with the form .

    Hi,

    Yes, you can request a phone assessment if that's what you'd prefer.

    You may find that you get a completely paper based decision done this time though, without the need for any verbal contact at all.

    The occupational therapist report should be a good source of evidence for PIP so I agree that should be sent along with the form.
