I am writing this as it may be beneficial to some. If you don't agree with a decision or are curious about what is recorded about you then I suggest that you make a request for full documents. I did this and am very glad that I did. It helped put decisions and wrong decisions into context and satisfied my paranoia that they were writing information and not telling me what it was particularly around LCWRA. After all, who makes these decisions, they don't know me, how can they assess me on one meeting or phone call and make assumptions about crappy evidence provided by GP for example.

Ok, before I start ranting, to say that I have spent this morning reading the documents and I am now clearer about the system too. It is fortifying when DWP contacts write accurately what I have said, that is what I am focussing on. It is by nature of their recording systems a very, very repetitive read and some things have been redacted which I suspect are where they have screwed up. I am tired, can't be bothered to keep fighting everything but I am pleased I did this.

TLDR: You can request for all your DWP held information. They are legally obliged to return it within one month. I received it via my email as a document attached pdf as I have informed DWP that I do not wish to have telephone calls only email contact due to my various conditions. I forgot I had done that and was paranoid that all my information had been posted to my neighbours.

    I was very specific about what information I wanted and the dates and the outcome notes recorded. I have done this a few years ago and just asked for all the information that they have about me which was massive, mostly pages with one number on and a waste of time as it wasn't the specific reports that I wanted to see. So, be specific.
