Please help to understand a Pa4 renewal rivew

Hi I really need someone's help please
I suffer with severe ADHD , anxiety, depression, OCD , number dyslexia and currently waiting for my face to face ASD assessment .
Last week me and my carer had a PIP review with a assesor regarding my mental health, asked for my PA4 arrived today my carer went through my report as I'm severely dyslexic I can't read or write and she has noticed a lot of errors and a lot of inconsistencies, there's consultant psychiatrist leter by the saying of the assesor with a date of 23/05/2027 which its weird because we are in 2025 , and she states the medication at the beginning ,and all through the comments she states " he was not taking mental health medication for this report " when I clearly do at a high dose . I suffer with a anxiety and depression which I'm on high dose as well .So my report was quiet negative but at the end it states " based on the claimant's likely future circumstances, it would be appropriate to review the claim in 2 years " wich we struggle to understand what that means .
And my carer also noticed that the assesor was a paramedic which from our understanding they only have basic training when it comes to mental health.
If someone could please help and give us some advice it will much appreciated

They seem to suggest there is a chance of some improvement - so consider 2 years an appropriate time to undertake your next review.
All assessors regardless of former profession, receive the same training within the health assessment agency.

Can I ask please, we are so confused we don't understand the descriptors why it's a tick in one of the boxes what would that suggest ?

For PIP if you are unable to do the activity reliably, repeatedly, safely and in a reasonable time frame for more than 51% of the time, you are treated as unable to do the activity at all.
