When you receive the letter you will have 3 months to claim. Once you submit your claim your Income Related ESA will continue for 2 weeks and then stop.
If any part of your ESA is contributions based that will continue but be deducted from UC.
As you’re in the Support Group then you will be entitled to the LCWRA element from the start of your claim. You will not need to be reassessed or need to provide any fit notes.
Citizens Advice have a help to claim service if you need assistance.

Hi @zigg I know it all seems a little confusing, I hope Poppy's answer has helped. You will find a bit more information about migrating to UC here:

I received the migration letter today .the thing that's bothering me is I renewed my esa about 5 years ago and my evidence is well outdated. will I be reassessed.?

Poppy is on a break from the forum.
No new medical assessment will happen as a result of the migration.
The application form will ask for basic details about your medical conditions.
Then once you've submitted your application ESA will confirm your award with UC.
