Change of circumstances PIP

I need to report a change of circumstances to about my mobility. How do I go about this? Is it only a telephone call? Do you have to tell them over the phone? What changes or do you fill a form in that they send out?
I’m very nervous on the telephone and would struggle to get everything right as I find it very intimidating talking to these people.

@kaylalala999 you can call or write to PIP about your Change of Circumstances. PIP will send out the PIP 2 form "How your disability affects you". You fill this form in like a new claim, including your worsening mobility. It will then be reviewed.

@kaylalala999 If you are already on Enhanced Rate for both elements, not a lot of point putting the Change of Circumstances as you can't increase your award!

I’m only on basic living at the moment. Realistically I should have been given basic mobility also but I was too nervous and didn’t tell them how bad I was. Now I’m much worse on my feet so they need to know.
