Sick not

hi there
I'm wondering if anyone can advise
I did my managed migration application, I am a carer who is also disabled.
On getting help doing the online application I filled in my health conditions. I was verified as a carer
I was then asked to provide a sick note.
I have not filled in dates of sick note because it does not have it. On fit for work
It states indefinatly
The date I was assessed is day I wet to gp.
I explained in my journal to work coach that my fit note has no dart and end date .

Can anyone advise. It only has the date of assessment on which is day I got my gp appointment.
I don't want to get into trouble putting a date . I am long term sick on pip
I would be grateful for any advice.

Hi @jen1827 it sounds like you have quite a unique circumstance. The response you got is a little confusing and doesn't really seem to clarify what you need to put there.
To be sure (because it's important you put the right information in that they won't dispute later), personally I would go back to them to reiterate you don't have an end date, because it states indefinitely, and ask what date you should put because you don't want to put an incorrect date?

I know you cannot get both carers and health as one will cancel the other out.
The way he answers is for us to consider the health element , where you can only get claim one or the other makes me feel it's optional and would not make a difference as they have verified me as a carer with carers allowance
I feel what's the the point
I dont feel there's anything to gain , but more stress

Hello jen, the sick note is now called a 'fit note' - a Statement of Fitness for Work.
The start date is the date of your GP's signature. Your GP has advised that "you are not fit for work".
Your GP has deleted 'will' leaving "I will not need to assess your fitness for work again at the end of this period". That's clear enough for DWP.
Report your fit note ending with a date in five years' time.

hi Holly I just thought I would share the fit note so you can see what I mean about the dates and also the Gp has not put a signature on it just his dr name and practice
Hope this help you see a clearer picture as I feel I don't always explain properly

Just to let you know they assessed me as a carer , although I put in my jounal and explained sick note. They have not responded and have gave me my first payment with transitional protection .
I am getting more than what I was getting before so really don't understand.
But I know there are other disabled carers and would love them to see this.
Just from someone else's experience.

They looked at my sick note and now they are saying I will recieve the uc50 to fill in
To be assessed for the health element.
I don't understand as I'm a carer for my disabled son
How will this effect me and will I loose my transitional protection
Also my caring for my son as there is no way I can leave him alone, he suffers seizures and has other disabilities

Because you reported your health problem UC have started the work capability assessment process.
If you are awarded LCWRA then your Transitional Protection will reduce.
You will also lose Carers Element as the same person cannot get both.
Hopefully, your Transitional Protection will reduce by the difference between LCWRA and Carers Element.
There was a court case because what used to happen was people ended up worse off because LCWRA reduced TP and Carers Element was removed.
Since the court case some people in the same situation haven't had the correct outcome so if that happens to you I would seek advice from a trained benefits advisor.
