PIP review, changes and unhappy with original assessment

Hi, need some advice completing my review form, this brief outline of previous assessment and change of situation.

Medical issues in original application



Bi Polar

Social Anxiety 

Note, diagnosed 30 years ago, never knew it was possible to claim

Hip pain:

On waiting list for right hip replacement, left is also deteriorating and will need replacing.

Lower back pain, been a problem for over 30 years, diagnosed with arthritis and diminished space in vertebrae. 

Dysfunctional right ankle, previous operation around 15 years ago to remove large bone spur and clean up joint due to arthritis.

Diagnosed arthritis in left ankle and left knee.

Plantar fasciitis in left foot.

Consistent pain and pins and needles in both wrists and hands, weakening of grip, numerous tests inconclusive.

Shoulders pain

Right shoulder, operation 15 years ago to repair rotary cuff, remove bone spur, still gives me some pain and not 100% functional

Left shoulder, severe pain and lack of movement. Despite telling Drs it felt exactly the same as right shoulder and needed operation i was told i needed steroid injections first, this was delayed over 2 years due to covid.

My mental health issues were rejected, unfortunately I was too slow to appeal.

The decision was made via online interview and took no notice of evidence presented from mental health professionals.

Bi Polar by its nature means i have good and bad periods, my social anxiety means i struggle while in groups of people, social situations. I manage for short period on one to one basis if there is purpose, such as speaking to Dr, remote conversation is also easier.

Despite telling the accessor I struggle in group settings and gave examples, she declared i was fit and able too.

I have not worked in decades and my wife supports me.

Changes since last assessment

Right hip replacement carried out in 2023, operation went fine, though change negatively affected my right ankle, i experienced severe pain in ankle and in outside of foot. 

Consultant is happy to replace my left hip when i am ready, but due to ankle issues i am reluctant and this would mean addition weight on my right ankle until left hip has fully recovered.

Right ankle, after seeing a consultant it was diagnosed that due to incorrectly set fibula bone that i broke in early 20s, my ankle is out of alignment by around 6 degrees, advance arthritis also confirmed. The hip replacement exacerbated my ankle misalignment causing me to put more weight on the outside of my foot.

Ankle Consultant reluctant to take surgery option due to my ability to heal due to possible circulation issues.

Both ankles have deteriorated, though Plantar fasciitis improved with insoles as has right foot outer pain.

Right ankle pain increased, movement and usage decreased. Walking is painful from the moment weight is applied, continued weight bearing results in swelling, stiffness and increased pain. Driving also causes pain due to constant movement from accelerator to brake, 10, 15min of driving before pain killers required.

Was prescribed custom insoles for both feet in 2024.

Operation in 2024 on left shoulder in July 2024, repair to full rotary cuff tear, removal of bone spur. Recently discharged from regular physio therapy care, though can return at any time if issues arise. The shoulder is better, but far from 100% still some pain, movement has some restrictions, but strength is an issue as there is a lot of muscle wastage due it taking over 6 years to get to surgery.

Pain is constant in wrists and hands, right elbow.

Investigation for Rheumatoid Arthritis in 2024 found no evidence of the condition, though the consultant did diagnose Fibromyalgia.

Recent investigation into right elbow pain, increase in pins and needles in right hand and numbness in outer two fingers confirmed diagnosed Cubital Syndrome, presence of bone spur. All four operations included removal of bone spurs.

Recently I have been, in addition to lower back pain, suffering back pain in the center of back and spasms, these affect my ability to move and walk.

I have also seen an increase in pins and needles in my legs, especially my lower legs along with bouts of numbness especially the outer toes of my right foot.

While discussing options for back issues with my Dr he suggested an MRI, I informed him i had one in 2024 for rheumatoid arthritis, he pulled up the MRI and mentioned I had had a prolapse L5. I told him this was new to me and I had never been diagnosed with one, but it would explain over 30 years of extreme back pain and spasms.

He has referred me to the advanced physio dept for back and right elbow, 6 weeks on i am still waiting for an appointment.

My lower back pain has increased dramatically, weight bearing, sitting, standing and walking are difficult, pushing through with pain killers just results in stiffness, more pain that takes days to settle down. Sitting is possible only in soft reclining seats, upright and hard seats cause pain.

I am currently on 30mg of amitriptyline, with zappain as required.

    What kind of help do you need with the form? Many people find that treating the review form as a new claim is a good way to do it.
