UC50 form help
Hi,
I wondered if anyone could help/guide me in the correct way to fill in uc50 form.
Medical conditions are
Iron defiency anemia-
Aniexty& depression
BVVP
I’ve suffered with iron problems since I was young. Medication doesn’t help I have iron infusions when they are low. In the run up to having an infusion I suffer with all of the symptoms of zero energy all the time, serious lack of motivation due to being so exhausted, dizziness and restless legs. These symptoms stop for around 3 weeks before they start to creep back up again.
Aniexty- I live with the feeling of Aniexty so anyone who suffers with this knows what I mean. Chest pain, churning feeling in stomach. Feeling hot and clammy. Legs are like jelly. When I was working I was dropped to and from work. My Aniexty attacks are too common and I’m afraid of one happening when I’m out alone and what could happen to me. I don’t go anywhere alone. Food shopping gets delivered and anything I need people will get for me I’m basically a hermit.
BVVP- this is fairly new condition currently waiting for an appointment with ent. EXTREME vertigo when rolling over in bed and lifting my head forward/backwards. Loss of balance. Nausea/vomiting. I have antisickness tablets which do help but doesn’t help the vertigo or loss of balance.
depression- truly believe this is linked to long term iron problems. 25yo and I feel like I have no quality of life. It’s just me inside a black tunnel. Don’t want to associate with people. Have lot of intrusive thoughts. There’s never a good day seems to be no light at the end of the tunnel.
I do not prepare foods due to intrusive thoughts. Do not handle hot pans etc due to dizziness and aniexty attacks. I can only work alone/ with 1 person although I cannot engage with them.
any help would be appreciated. Xx

Hi @chlooooxxx If you're hoping to be put into the LCWRA category, it's worth having a look at the descriptors and seeing where your conditions might score points:
As you're filling in the form, try to give some examples of how your conditions have affected your ability to do what they're asking about. If you run out of space you can add other sheets, just make sure you label them with your name, national insurance number and note on the form where they can find the extra information.

Thankyou both so much. I’m having a pain with post in my area at the moment. Do yous know what colour letter I should be expecting and if it says UC on it?
