Hello. I was wondering if anyone could tell me,how I go about asking my doctor for medical evidence to support my MR PIP claim please. Thank you

I have seen on another site,a form for your GP to fill in .It supposed to be for pip.
You have to be a member which I think the charge is £19 ? But it will let you assess files ect to help.
I think £19 is a lot of money when you are on basic benefits.Though if I could afford it ,I would !
But then it's a choice between heat electric or food to cut for 2wks.0 -

I can't afford to pay it,but stressing if I do it could make a difference to getting pip.
But I think it is wrong to pay for information about us .0 -
I had to pay £20 for a doctors letter stating that I couldn' walk more than 5meters they then took my standered mobilty away and gave my nothing1
