Form filled inaccurately by benefits advisor welfare charity

Help to put my issues right this is the only service in plymouth and they have basically ignored my requests to get a second opinion on my ar1 coc pip form to be looked at by anyone to double check it was done right as I no she didn't write half the things I said dismissed me.. didn't look at my ongoing award answers I just wanted to get it looked at. The manager rang after 3 weeks wasn't kind hadn't looked at it, who do I turn to I'm so ill.

    Hi @scared56 I'm sorry you've had such a bad experience. Do you have any friends or family that could look over the form for you?

    After an awful Christmas and new year I contacted CABas they sent me to them they just said sorry, I just can't believe how she made me feel, I dod get a friend to write another whole form which I feel after always having that advice service we filled in much detailed than any other clain form ever, but I think they may find that inconsistent but my friend neighbour was looking up what it all meant as I couldn't of, she sent it and it's with serco now, I havnt they just judge cause I'll sound fine but since October I had literally every weekend 48 hours no food or sleep cause its takes overall bit I won't have anyone on assessment don't no what I'll do

    mental health since 2008 and claimed DLA it said I lost my job due to mental health, and I was prescribed controlled drug g and antidepressants, if I mentioned I'm dealing with a substance misuse centre to manage suicide attempts and alcohol use, should I send evidence of long suicide suicide history and include gp letter of same conditionconditions to prove I had mental health issueissues didn't cause mental health issues it was a block.. or not bother I just don't want any more money just a longer Award to get through all these massive changes

    I'm so sorry for everything you're going through @scared56. I'm glad your friend was able to help you with the form. It's completely understandable to worry about the result but remember that if they take it away you can ask for a Mandatory Reconsideration. I know it's not the ideal situation but it's there if needed.

    It's always good to send in as much evidence as you can to support your claim. Just make sure your name and National Insurance number is written at the top of each page.
