I haven't been in that situation before, but I think that if you don't complete the forms they'll just stop your payments. I really hope that you are better enough by review time but make sure you claim what you're entitled to, you shouldn't feel guilty.

Some people have a 'marker' on their claim if they were previously considered to be vulnerable from past assessments. For these cases, PIP payments will not just stop if the forms are not returned. The DWP may keep trying to contact the claimant until they get a response. For 'normal' claims, PIP will just end if the forms aren't returned on time, the DWP will send a letter to confirm this if it happens.
