PIP AR1 Review Form confusion

Hello everyone

I got my PIP AR1 review form today in the post and already filled it in but I'm a bit confused what I send back.

Do I include the first few pages(the letter to me saying about my review is due with name and address etc and the guide on how to fill the form in)?

Or do I just send the part that's marked as an AR1 that I filled in?

I phoned PIP but they wasn't sure and I could send it all in but this might cause issues for the assessor reading it.

Any help would be amazing

    Managed to work it out.

    For anyone who's looking for answers, you remove Page 1 to 6 and post back 7 onwards
