What if I dont consent to gp returning uc113 form?

Hi i got a text from gp surgery asking me for permission to fill and return uc113 form from dwp. Problem is I have no diagnosis and not much record as ive only recently started seeing gp about this condition but im hoping to get into substantial risk category which apparently doesnt need a formal diagnosis. I also dont know this gp, ive only been once and dont think i met the gp wholl be asked to return the form. What do you think - would it be better to ignore the text and let the deadline pass? As far as dwp is aware i dont have a phone so i can say i didnt get the text.

You do not have to agree with your GP sharing information with the DWP and you have the right not to consent to your GP completing the UC113 form.

If you don't consent, the DWP will usually make their decision based on the information they already have. The form is generally a factual summary taken from your medical records rather than a detailed opinion, so it can help support what you’ve said in your questionnaire.

If you’re worried because you don’t know the GP well, you could speak to the surgery about your concerns or ask for copies of any relevant medical notes or letters to send in yourself, so the DWP still has clear evidence of your situation.

Thanks. Ive given relevant hospital records. Not sure what else the gp can add except that ive seen them about it. I feel like the lack of a diagnosis wouls only undermine my claim

Yes, a completed UC113 would be good medical evidence to support your claim. As Adrian has written -

The hospital records you submitted won't be seen by a health assessor but should be taken into consideration by a Decision Maker along with the UC113. This will also include any medication used to treat your condition(s) and psychiatric history if relevant.

No, your claim won't be rejected just because they do not get the UC113 back, but allowing information sharing can make it quicker and support your claim.
