Hi,

Well the dreaded envelope dropped through my letter box today. I feel sick not eaten and just cried even to the point told my hubby I can't go through this again.

I've looked at the form and I have a couple of questions. On each question can I put on the form see attached because I'm better putting my reply down on one piece of paper rather than breaking it down on each section on each question, if that makes sense.

Also can I leave questions out that don't relate to me because the last 2 reviews I've filled sections in and have got 0 points. I would rather focus on questions that relate to me.Tyia

    Hi @snuggles65 - yes, please do as you suggest. There's not much room on the form, so do just add paper (with your name & National Insurance number at the top of each page), & so long as you say which question it relates to answering there will be fine.

    For activities that are not applicable, just tick the 'No' box.

    Do try to give as much detail as you did with your initial claim form trying to give a couple of recent, detailed examples of the difficulties you face doing/attempting any applicable activity/descriptor. Also mention any untoward consequences of doing so.

    hi as above definitely use separate papers for full and detailed answers, I put "see enclosed" on the form itself. on the separate sheets I referenced everything with the question numbers and activity headings from the form so they could see where I was answering what. then they don't have to ferret around for the answers. And put national insurance number on all pages including evidence.

    I just stated on the form the activities I was ok with.

    Hi

    I'm going to do that l will only answer what they are asking on each question but leave the out that are not relevant to me or reading and speaking, then on a separate piece of paper I am doing a full blown essay 😂 😂 telling them how each activity and how it impacts my day to day life. I will also do a personal statement at the end 😂 they are going to love me when they get it back. I do this because the only evidence I have is my GP, prescription, letters from family and diaries. I've never had trouble before been on pip since 2013 and DLA before that, only difference I was awarded DLA indefinitely put now I'm reviewed every 3/4 years.
