Struggling to find work

I was diagnosed with Autism Spectrum Disorder last year at age 24. I am looking to live on my own soon, however I have little luck with finding work. I have work experience but it is for short periods at a time, I get no reply from CV's I send 98% of applications, and if I do get an interview I practically never get offered the job because I struggle with eye contact and talking about myself. I would just like someone to give me a chance to show I can do things, even though it might be different to everyone else. Also what sort of support could be available to me? Thanks
