Hi, I am looking for some advice my brother cannot, read, write, count or tell the time he got 2 points for Needs prompting to be able to read or understand complex written information.
But go 0 points for Cannot follow the route of an unfamiliar journey without another person, assistance dog or orientation aid. How can he plan and follow the route of an unfamiliar journey if he cannot read.

Depends how old he is.
Does he need to plan a route
Does he have a carer
Does in live in a home
Loads of things make a difference to what points you get0
