Mandatory Reconsideration

Sorry for being such a pain and asking so many questions. I had a benefits adviser come to my house today to help me complete a mandatory reconsideration form. We used the award letter as a guide to complete the descriptors that I want to challenge. However, I now think that maybe I should also include some of the assessor’s report that is wrong ie she said that I daily walk the short distance to my local shop which is not true as I can only manage it occasionally. She said I didn’t need help in the kitchen but I use a perching stool. I always need assistance in the kitchen due to painful and weak hands, wrists and fingers etc. I drop pans, can’t open packets or cans. I’ve often burned or cut myself trying. I showed her the scars from my burns but she said no  injuries reported. She said I could use a mobile phone so I don’t have dexterity problems but I use a ‘phone pen’ to touch the keys and when I want to type a message I dictate into the microphone. I don’t want to criticise her or the report but I want them to know the truth. I want to explain what my rare illnesses are and how they affect me. She admitted that she had never heard of two of them. Should I send a letter with the form explaining everything or just send the form? TIS
