How can I get ta motorised wheelchair?

Hi, I am Bintoo Singh. I want to discuss about motorised wheelchair. How can i get this wheel chair from the Government,  I am doing job in private sector gurugram. Current address Delhi, permanent address Uttarakhand Haridwar. I am facing some problem to reach one place to another because i am 80 percent disable by both legs. Any one show me way to get motorised wheelchair.

    Have you had a wheelchair assessment done by the NHS?
